One form should be completed for each student.  This form must be completed AFTER using the electronic payment link for mandatory and optional school fees.  Once payment is processed, an order number will be sent to you from 'OSP Admin' to enter as you complete this google form.

If you cannot use the electronic pay link, this form should not be completed and your process will become manual. Visit Crabapple beginning  July 1, 2019, Tuesday, Wednesday and Thursday from 8:30am-3:00pm to pick up your Golden Ticket. We encourage everyone to take advantage of the ease of electronic payment thus enabling the completion of this form electronically.

This google form provides an electronic opportunity to pre-register and confirm your on-line purchase of start up fees before the Open House day.  When you pay the return to school fees, please print a copy of the confirmation receipt and bring it the school in exchange for your Golden Ticket, prior to Open House. Electronic versions (on your phone) will also be acceptable.  By taking this simple action, you will be able to advance quickly to other areas of the open house and avoid lines to pay fees:

All Rising 6th graders must provide two forms of residency verification, in order to receive a Golden Ticket.
