This registration form will be used for all
NEW Choice Registrations for the
2024-2025 school year.
New Registration is defined as any child hoping to attend a new school, regardless of prior participation in the Choice Program.
All Choice Renewals (students hoping to keep their current Choice placement) will be contacted via phone call during the month of February. If your child is currently in the oldest grade in their school (3rd, 5th, 6th, or 8th) you will have to reapply for Choice for a new campus. If you did not receive a phone call, please call 318-442-8891 to verify your intentions.
You will complete one form PER child.
