The Garland ISD Athletic Department would like to thank you for taking part in our new process of submitting athletic pre-participation paper work online. This automated process will allow us to be more efficient in handling the forms of our student athletes. ALL forms will be submitted in this manner except for the UIL Physical and Medical History.
Instructions: UIL Pre-Participation Physical & Medical History
1. Click on the tab titled “Download and Print”.
2. Click on “Pre-participation Physical Evaluation”
3. Fill out all highlighted areas on both the Medical History and Physical Evaluation form and then print.
4. Take the forms with you to the doctor to obtain your physical.
5. Turn in to your Athletic Trainer or Coach.
** Note for physicals: All physicals should be completed on an annual basis and on the most current UIL Pre-participation Medical History and Physical Form (Revised 2020) for the upcoming school year.
To access a copy of the UIL Safety Training form, please click the Download and Print tab on the right-hand side of the page once you proceed with online forms.

Instructions: GISD & UIL Pre-participation Forms:
You will need your son/daughters full name and their school ID number to complete the forms.
1. Click on the tab titled “Electronic Participation Forms”.
2. There are 8 forms that you will need to complete and submit electronically. They are:
• UIL Acknowledgement of Rules
• UIL Concussion
• UIL Steroid
• UIL Cardiac Awareness Form
• GISD Emergency Medical Release
• GISD Athletic Code of Conduct
• GISD Athletic Policies
• GISD Student-Athlete Safety Training Acknowledgement Form
3. Click on each form and complete all required information.
4. If there is information that you cannot provide, example; home phone number, put “NA” for not applicable.
5. Sign the online form: click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.
6. Once you have filled out the forms (both parent and student), you will have the opportunity to
print the form. This is for your record only, there is no need to turn this in.
7. Enter a valid email address at the bottom of the form an you will receive a confirmation email once the document has been submitted and received by the Rank One system.
8. Click on the “Submit” tab at the bottom. You will receive a confirmation email that it has been received.
Should the document not submit, scan through it looking for areas in red that say “Required”.
**Note: ALL forms must be submitted before your son/daughter will be allowed to participate in athletic practices or games.
