Homeless Persons' Memorial Day Registration Form
Please join us to honor the lives of individuals who passed away while unstably housed, or due to the stressors of homelessness, this year in New York City while recommitting ourselves to the fight to end homelessness.

The observance will take place on December 15th, 2022 at Fifth Avenue Presbyterian Church: Bonnell Hall 7 West 55th Street, New York, NY 10019. The program will start at 5:30 PM. Snacks will be provided at the beginning of the program and a to-go meal will be provided at the end. Due to the ongoing COVID-19 pandemic, masks will be required so we can try to keep everyone safe and healthy. A zoom link will also be provided for those who would like to attend virtually.
