Thank you for joining us for this year's Leader in Me Night! A parent or guardian must fill out this form. You may submit one form for your entire party.

Please note:

A freewill donation supporting Leader Valley may be made at the door but is not required.
Food and snacks are available, but quantity is limited.
Swimwear is required for the pool. Because space is limited, the pool will have two shifts: 5:00-6:15 and 6:45-8:00. There will be space for 120 people during each shift.
A parent or guardian must be present with students under 13 years of age.
