Malama Pu'uloa Registration & Waiver 11/12/22
I, the undersigned, desire to participate in the volunteer community service project, at Kapapapuhi Point Park with Malama Pu'uloa. The outdoor project work may involve walking/hiking to site, lifting objects up to 50 lbs, and working with sharp tools or trash.

I, the undersigned, acknowledge that I am aware that in participating in the above-mentioned volunteer community service project, that  there may be risk for injury to me, but that I knowingly and with full knowledge of the possible consequences of injury waive any and all claims against the Rotary District 5000 and all individual Rotary Clubs as members of District 5000, for personal injury and/or bodily injury that I may suffer  as a result of participating in the service project. I further agree to hold harmless the Rotary District 5000 and all individual Rotary Clubs as  members of District 5000 for any damages or injuries resulting from any acts or failure to act on my part during my participation in this service  project. I acknowledge that I may at any time terminate this voluntary participation in the service project. I also do hereby approve the use of pictures, videos of myself (or listed participating minor) for this service project.

I knowingly sign this waiver to induce Rotary District 5000 to permit me to voluntarily participate in the service project. This form will also act as our CONTACT TRACING FORM.

SERVICE PROJECT DETAILS:

 We are so grateful that your group (or you as an individual) are coming to Kapapapuhi Point Park to be a part of Mālama Pu`uloa - restoring Pearl Harbor! Please share the following information with your group and if you have any further questions/concerns, do not hesitate to contact us at malamapuuloa@gmail.com. Work days are from 8am-noon

2. Kapapapuhi Point Park is off of Fort Weaver Road in the West Loch Estates neighborhood - MAP APPS DO NOT PROVIDE ACCURATE DIRECTIONS - turn on Kapapapuhi Street to access park, see direction photo below
3. We meet in the parking lot and there are restrooms and potable water 
4. Please BRING:
     *Your own water bottle
     *Sun protection
     *Work gloves
     *Your own hand sanitizer/wipes for safe public restroom use
     *Closed-toed shoes you donʻt mind getting muddy (rubber boots/military style boots/tabis are great)
     *Bringing a plastic bag for muddy shoes
     *Clean slippers for after
     *Wearing long sleeves and clothes you donʻt mind getting muddy - perhaps dark colors 
5. There are a variety of work levels including lighter but important work to accommodate a variety of ages, fitness levels, etc. We are currently focused on our Honouliuli Stream/fishpond site engaged in clearing invasive mangrove, preparing planting areas and planting native species. 
6. We serve pizza for lunch at our Kapapapuhi Community Work Days and always have plenty of ice cold water to refill our volunteerʻs water bottles. 
7. Consider preparing for cultural protocols - invite those of your group who do not know E Ho Mai and Mele Honouliuli to learn them - words/recordings are available online… https://cutt.ly/6aHwCNI . If those with ancestral roots to Pu`uloa/Honouliuli are present in a group we host, we humbly defer to cultural protocols they wish to practice.
