SUHSD On-Campus Athletic Physicals
The SUHSD on-campus athletic physicals will be held on Saturday, March 20th, from 11pm to 12pm at the Olympian High School Gymnasium, and will be completed by a registered nurse. Please arrive 5 minutes before your appointment window. The cost is $25 (CASH ONLY). Masks are REQUIRED to be worn at all times! All students must complete a COVID-19 screening questionnaire and get a temperature check upon arriving on campus.

You must have the SUHSD Physical Form with the Parent Consent portion of page 1 and the Health History questionnaire ALREADY FILLED OUT. If you need the form, please click the link below:
