Louisburg High School STEM Academy Application 2019-2020
This is the official application for students interested in joining the Louisburg High School Science, Technology, Engineering, and Mathematics (S.T.E.M.) Academy for the 2019-2020 school year.  All parts of the application must be filled out in order for the student to be considered for the academy.  This information, as well as attendance, grades, and behavior, will be used to determine acceptance into the academy.

Student Agreement
I am interested in enrolling in the Louisburg High School STEM Academy and want to be considered for admission. I certify, the information that has been provided is complete and accurate. Incomplete and inaccurate information may be grounds to delay or deny admission.

If accepted to the academy, you commit to do the following:
o Abide by all the rules set forth by the Louisburg High School and Louisburg High School STEM Academy
o Make every effort to be prepared, present and attentive during learning sessions with appropriate behavior.
o Put forth a high level of effort and a positive attitude towards all learning
o Accept challenges and high expectations of achievement.
o Work collaboratively when necessary and aid others when needed.
o Work to build and maintain a positive learning community by honoring all learners and the learning environment.
o Be an active participant in all field trips an./or presentations provided by the school/district.

It is YOUR responsibility to be prepared for learning and that you must seek to solve any problems that arise in that process. You also understand that the students of the Louisburg High School STEM Academy work together as a learning community, and you freely chose to engage in this community as a servant leader. You understand that projects, tasks, reading, writing, thinking and speaking are regular requirements and you must support the ways we are expected to learn.You understand that you are required to participate in the on-campus STEM Expo and create a STEM Showcase related to the Project-Based Learning (PBL) projects that you complete.

You also understand that failure to fulfill the terms of this commitment and maintain passing grades (70 or higher) could result in withdrawal from the Louisburg High School STEM Academy.


Parent Agreement
I confirm I am the parent of the interested student and agree to commit and adhere to all campus and district policies. Students will be expected to participate in service projects and volunteer to improve their community. Parents are expected to be an active partner in their child’s education and will be asked to help with a variety of projects and activities.

Parents and student understand that, as a school of choice, continued or serious violation of district, campus or program policies my result in the student’s dismissal from the STEM Academy.

The STEM Academy is rigorous, and students will be expected to be present, prepared and participate in the classes. Students with academic problems will be expected to take full advantage of available interventions for support. Those who choose not to take advantage will risk possibly being removed from the program.

As a technology -rich program, I understand my child will need access to the Internet. Communication between the academy and home will often be through text or email (which requires internet access). (*Please be aware that the campus library and STEM classrooms are often open before and after school for student access.)

I understand that there it a $25 technology fee for the Chromebook. I also understand that each student is responsible to participate and raise at least $150 in our fundraiser to assist in the cost of technology.

*Rising ninth graders who are districted to LHS may apply for the LHS STEM Academy.  Rising ninth graders who are districted to BHS and FHS may also apply.  

** Bus transportation will not be available for students from outside of the LHS district who are accepted into LHS STEM Academy; parents/guardians will be responsible for arranging transportation to and from school.  Once accepted into LHS STEM Academy, parents of students outside of the LHS district are to submit a Request for Student Reassignment form along with the STEM Academy acceptance letter to Diane Marks, Office of Student Assignment, by May 31 for the upcoming school year.  This form must be submitted each year the student is enrolled in STEM Academy and lives outside of the LHS district.
