2021 TRAILBLAZERS AWARDS NOMINATION FORM

The Santa Cruz County Women's Commission's Trailblazers Awards recognize women in Santa Cruz County who, through their professional or personal efforts, are making extraordinary differences in the lives of women and/or girls, as well as inspiring and encouraging them.

Please be as complete and specific as possible when completing this form and provide as much information as you can. Awardees will be chosen based on the information you provide.

Trailblazers Awardees will be recognized during the AUGUST 24, 2021 Santa Cruz County Board of Supervisors meeting, followed by a special recognition gathering on WOMEN'S EQUALITY DAY, THURSDAY, AUGUST 26, 2021.
