Please take a few minutes to complete this short survey from the Colorado Health Foundation. As a member of a coalition that received a Community Food System grant, your feedback can help the Foundation learn more about:

Efforts to change the local food system in your community, and
How people who have experienced hunger have taken part in those efforts.

Survey results will be used for future planning efforts. Results not be used to assess your coalition or make funding decisions for your coalition.

We want your honest feedback. Your answers are confidential. This survey does not ask for any information that could identify you. If there’s a question you do not want to answer, you can skip it.
