The responsibility of your Counselor and/or Teacher in preparing the “Secondary School Report” or letter of recommendation for colleges and universities is to provide a summary of achievements beyond your academic record. Conveying your unique qualities and potential for growth is not an easy task. Therefore, we would appreciate your giving us an honest estimation of yourself: what you have done, and what you have left to do, both in school and any other part of your life.

If you'd like to provide additional info you feel would be helpful, including student resumes, college essays, or personal statements, please e-mail those directly to your counselor.

School Activities: List the activities in which you've participated, number of years and the amount of time per week you spent, and what you have gained or learned from each activity. Consider artistic, athletic, club, leadership, and community activities.
Which of these activities was most important to you? Why?
Non-School Activities: What do you consider your most important activities outside of school? List jobs (paid or voluntary), religious activities, hobbies, travel, music, art, and drama. Include the number of years of your involvement and the amount of time you spent on the activity weekly, and explain why this activity was significant to you.
Inside or outside of school, which awards and honors have you received? Which elected offices have you held?
Describe the academic accomplishment (major paper, science experiment, and/or artistic project) you are most proud of, and tell why you take pride in it.
List your three most distinguishing or admirable qualities. Explain each with several sentences.
List your academic strengths and give at least one example. (Be specific--e.g. problem solving, writing, creative thinking. Do not just list your strong subject areas.)
What do you hope to accomplish in college and after? Consider your career goals and your broader goals.
If you feel that your academic performance in high school is not an accurate reflection of your ability, please explain any circumstances that might have interfered with your academic performance (e.g. illness, home responsibilities, language barrier, or learning issues).
Describe how you overcame a setback or disappointment.
Where do you see yourself in 5 years (e.g. degree you plan to earn, career you plan to have, graduate degree you plan to pursue)?
How are you unique? If your counselor just arrived at Milton this year and met over 400 students, how could she remember YOU?
