Instructions

Select one option from the "Nomination Award" drop down menu.
Fill out all information in the Nominee and Contact Details section.
Answer all questions under the "Submission" heading
NOTE: Each question has a limit of 700 words. 200-500 is ideal.
Upload your file attachments
Each attachment must be no larger than 3MB and a maximum of 10 attachments applies.
If you cannot upload your attachments, please email them to nswyaa@awardsaustralia.com
Tick the "I accept" box and click the "Save Draft" button.
Click the Submit button only when the nomination is 100% complete.

IMPORTANT NOTE: Please save your nomination every 10 minutes. The page can time out and information can be lost. Tick the "I Accept" box and click "Save Draft" each time.
