This questionnaire is part of the TransForm Project, a Schweitzer Fellowship project through the Pride Center of Vermont. The TransForm Project hopes to reduce transgender isolation and discrimination in Vermont, and to help trans and gender nonconforming Vermonters navigate transition processes more easily. The goal of this survey is to learn about barriers to transition processes (like name and gender marker change), as well as to gather stories of success during and after transition in Vermont.

Your participation in this questionnaire is completely voluntary. Your name, date of birth, or other identifying information will not be collected, nor will details about personal medical information be obtained.
