Eco Rotary's Tracks Beach Cleanup Service Project Waiver Form 08.27.22
I, the undersigned, acknowledge that I am aware that in participating in the below-mentioned volunteer community service project, that there may be risk for injury to me, but that I knowingly and with full knowledge of the possible consequences of injury waive any and all claims  against the Rotary District 5000 and all individual Rotary Clubs as members of District 5000, for personal injury and/or bodily injury that I may suffer as a result of participating in the service project. I further agree to hold harmless the Rotary District 5000 and all individual Rotary Clubs as  members of District 5000 for any damages or injuries resulting from any acts or failure to act on my part during my participation in this service  project. I acknowledge that I may at any time terminate this voluntary participation in the service project. I also do hereby approve the use of  pictures, videos of myself (or listed participating minor) for this service project.

I knowingly sign this waiver to induce Rotary District 5000 to permit me to voluntarily participate in the service project. This form will also act as our CONTACT TRACING FORM to meet the requirements of the City & County of Honolulu's Department of Parks and Recreation.

DETAILS: 
Eco Rotary Club of Kaka’ako has partnered up with Surfrider Foundation Oahu for this Tracks Beach Cleanup! Our Eco Rotary Club will plan to have a Social Beach BBQ day after the clean-up. Bring your swimsuits, snorkeling gear, surfboards  and food to share. We will have a BBQ to throw your food on the grill! 

Surfrider is the lead and we are here to support them! The Surfrider Team begins event setup 30 minutes before, they can always use the extra help.
AT EVERY CLEANUP:
-Surfrider manages all aspects of the cleanup from requisite permits and insurance to post-event waste management.
-Surfrider provides all necessary equipment including gloves, bags, trash-grabbers, hand sanitizer, sunscreen, and hydration station.
-All participants must register for the event event with Surfrider beforehand, and select “Oʻahu Chapter” in the drop-down menu of the waiver. Make sure to click here to register: https://x.gldn.io/e/7YLa9mhfZrb 
-Because plastic reduction is one of our main initiatives as an organization, we kindly ask that if you bring food + beverages, that you abide by our no-single-use plastic guidelines (this means NO single-use plastic packaging, cutlery, bags, bottles, etc.) That said, we are happy to provide you with a list of approved local Ocean-Friendly Restaurants.

LOCATION:
Our cleanup site is at Tracks Beach, we will plan to meet in the main dirt lot off the highway. Here is the google maps link: https://goo.gl/maps/dVWG6QzBWV7rUiLz5 . 
Our team will be excited and ready to greet your group underneath the big blue Surfrider tent at 10 am.

*REUSABLE WATER BOTTLE (hydration station provided)
*COVERED SHOES (Lots of Keawe Thorns)
*HAT
*Your own reef-safe SUNSCREEN (will also be provided)
*Your own GLOVES (will also be provided)
*Your own SNACKS (will also be provided)
*Your own picker/tongs (will also be provided)
* We suggest comfortable clothing suited for physical activity
*Swimsuit, beach stuff, snorkel gear, board and potluck dish/something to throw on grill to share for the Eco Rotary Beach BBQ Social to follow.

TIMELINE:
9:30am: Setup, come early to help!
10 am: Welcome!
10 - 10:30 am: Introduction to place, Surfrider Oʻahu, & Beach Cleanups 101
10:30-11:30 am: Letʻs clean this beach!
11:30 am-12 pm: Reflection of the day & what we found
12pm-2pm: Eco Rotary Beach BBQ Social
