Mithoff Program
The Mithoff Pro Bono Program offers limited free legal services to individuals who meet intake requirements. Each request for help is subject to initial review by the Mithoff Program.

By completing this survey, you request assistance from the Mithoff Program Gender Affirmation Project. All information you provide will be handled confidentially and securely.

Completing this survey does not in any way guarantee that we will be able to help you.

VIRTUAL CLINICS
At this time, we are only offering virtual clinics via Zoom. Clinics will be scheduled throughout the fall and spring semesters; we hope to offer in-person clinics when it becomes safe and possible to do so.

You may complete the survey any time, and if clinic dates are not yet scheduled, you will be notified about dates as soon as they become available. Make sure to add notifications@grow.clio.com and genderintegrity@law.utexas.edu to your safe senders list.

We are able to offer limited appointments at clinics. If we are able to offer assistance, we will email an offer of an appointment at the next available clinic. We also keep a running waitlist and will notify you of future clinic dates. You will not need to resubmit the intake form if you are scheduled for a future clinic date.

Please follow us on our social media pages to be notified of future clinics.

About the Project
The Gender Affirmation Project provides free legal assistance for those seeking to legally change a name and/or gender marker and update government-issued identification documents. In Texas, this requires a court order signed by a judge. Law student volunteers, supervised by practicing attorney assist applicants in preparing court documents - a Petition asking the court to grant a legal name and/or gender marker change, and a proposed Order which a judge may sign to grant the change. Once applicants obtain signed court Orders, they may use those Orders to update various forms of identification on their own.

Seeking Assistance at Name and Gender Marker Clinics
By completing the survey, you authorize the Mithoff Pro Bono Program to review information you provide in order to prepare court documents to change a name and/or gender marker on your behalf.

Our services are free, though there are costs associated with updating documents such as a driver’s license, birth certificate, passport, etc.

If we are able to offer assistance, you will receive a supplemental questionnaire and an Applicant Statement of Understanding via email to sign through Docusign.

You must sign via Docusign to participate in a clinic. If we are able to assist you, you will receive an invitation to our next available clinic date.

To acknowledge you understand these instructions and seek assistance from the Gender Affirmation Project, select “I understand and agree to the instructions above” in Question 1.

Please provide your chosen first and last name and email. All other fields in this section are optional.

If petitioning for a child, please list the child (preferred name) as the contact, but include contact information for the parent(s) or guardian(s).

The Applicant to the project is the person seeking a legal name and/or gender marker change.

Note: Texas does not recognize nonbinary or third gender markers on legal documents [i.e., you may only correct your documents to read “male” or “female” – some other states allow a third option, such as an “X”]. We hope this will change!

No problem if you have - you can complete it again! We just need to make sure we don't schedule you twice.

If you have a DPS State ID, enter the last three digits and note this is a Texas DPS State ID.

If you're an adult filer, head straight to Submit!

This could be, for example, a court order in a divorce that sets child custody and/or visitation, or a previous name and/or gender marker change order.
